What is a return to work form?
When your employee has been off work with sickness, sometimes it's necessary to carry out a return to work (RTW) meeting. This meeting gives you an opportunity to discuss the absence with your employee in person and find out if they need any extra support from you.
Whatever the reason for your employee's absence, whether it's physical or mental health related, keeping a record of it on file is important. You may need to refer to this form later on if you need to make reasonable adjustments to support your employee with a health issue - or if you need to send a letter of concern about repeated absences from work.
In this form, you can outline any next steps you or your employee plan on, or already are, taking to help manage the issue and prevent the need for further absences down the line.
